3. Before you mail it

  • White unruled card or paper, at least 4 x 6 inches, unfolded
  • Ink: Ink color not specified; blue is the safe choice
  • One card per outer envelope
  • Mail from the same state as your verified address
  • Each mail-in (AMOE) submission is valid for 60 days from the date you last logged in to your account.
